একজন ডাক্তারের কাজ কি? একজন ডাক্তারের কাজ হলো রোগীর রোগ নির্নয় করা এবং চিকিৎসা দেওয়া ৷ WHO এর ঘোষণা অনুসারে ফিজিওথেরাপিষ্টের কাজ বা ভূমিকা হলো , ” Physiotherapists assess, plan and implement rehabilitative programs that improve or restore human motor functions, maximize movement ability, relieve pain syndromes, and treat or prevent physical challenges associated with injuries, diseases and other impairments.” অর্থাৎ, একজন ফিজিওথেরাপিষ্ট মাংশপেশীর কাজের উন্নতি ও পুনরুদ্ধার, চলাফেরার ক্ষমতা বৃদ্ধি, ব্যাথা নিরাময়, ইনজুরি এবং অন্যান্য বিকলতা সম্বন্ধীয় শারিরীক সমস্যার প্রতিরোধ এবং চিকিৎসার জন্য অ্যাসেসমেন্ট (রোগ নির্নয়) করেন, পরিকল্পনা করেন এবং পুনর্বাসন প্রোগ্রামগুলো প্রোয়োগ করেন ৷ এবং WHO এর ওয়েব সাইটে দেওয়া তথ্য অনুসারে ফিজিওথেরাপি একটি সতন্ত্র পেশা এবং একজন ফিজিওথেরাপিষ্ট ফার্স্ট কন্টাক্ট প্রাকটিশনার ৷ অর্থাৎ, একজন রোগী কোন প্রকার রেফারেল ছাড়াই সরাসরি ফিজিওথেরাপিষ্টের কাছ থেকে চিকিৎসা নিতে পারবেন ৷
আমেরিকাতে ফিজিওথেরাপিষ্টরা সাধারণত ফিজিক্যাল থেরাপিস্ট নামে পরিচিত ৷ আমেরিক্যান ফিজিক্যাল থেরাপি এ্যাসোসিয়েশন (APTA) অনুসারে, “Physical therapists (PTs) are health care professionals who diagnose and treat individuals of all ages, from newborns to the very oldest, who have medical problems or other health-related conditions that limit their abilities to move and perform functional activities in their daily lives.” অর্থাৎ, ফিজিক্যাল থেরাপিস্টরা নবজাতক থেকে শুরু করে খুব বৃদ্ধ পর্যন্ত সকল বয়সের রোগীদের সাস্থ্যগত এমন সমস্যা যা তাদের চলাফেরা এবং কাজ কর্মের ক্ষমতা কমিয়ে দেয় সেসব রোগ নির্নয় এবং চিকিৎসা করেন ৷
অস্ট্রেলিয়ান ফিজিওথেরাপি এ্যাসোসিয়েশনের ফিজিওথেরাপি সংঞ্জা হলো, “Physiotherapy is a healthcare profession that assesses, diagnoses, treats, and works to prevent disease and disability through physical means. Physiotherapists are experts in movement and function who work in partnership with their patients, assisting them to overcome movement disorders, which may have been present from birth, acquired through accident or injury, or are the result of ageing or life-changing events.” এর মূল কথা হলো, ফিজিওথেরাপি একটা হেলথ কেয়ার প্রফেশন যেখানে বিভিন্ন রোগের এবং শারিরীক অক্ষমতার অ্যাসেসমেন্ট, রোগ নির্নয় এবং ফিজিক্যাল মেথোডে চিকিৎসা করা হয় ৷
ওয়ার্ল্ড কনফেডারেশন ফর ফিজিক্যাল থেরাপি (WCPT) এর ঘোষনা অনুসারে একজন ফিজিওথেরাপিষ্টের দায়িত্ব হলো –
Physical therapists are qualified and professionally required to:
★undertake a comprehensive examination/assessment of the patient/client or needs of a client group
★evaluate the findings from the examination/assessment to make clinical judgments regarding patients/clients
★formulate a diagnosis, prognosis and plan
★provide consultation within their expertise and determine when patients/clients need to be referred to another healthcare professional
★implement a physical therapist intervention/treatment programme
★determine the outcomes of any interventions/treatments
★make recommendations for self-management.
যার মূল কথা হলো, একজন ফিজিওথেরাপিষ্ট কোয়ালিফাইড এবং তাঁর দ্বায়িত্ব হলো, এসেসমেন্ট করা, পরীক্ষা নিরীক্ষা করা, রোগ নির্নয় করা, ফিজিক্যাল মেথডে চিকিৎসা দেওয়া, প্রোগনোসিস মনিটরিং করা, কাউন্সিলিং করা এবং প্রয়োজনে অন্য থেলথ প্রফেশনালদের কাছে রেফার করা ৷
উপরের রেফারেন্স গুলো লক্ষ্য করলে দেখা যায় যে, ফিজিওথেরাপি নিয়ে বিভিন্ন রকম কথা থাকলেও দুইটি কথা সকল যায়গাতেই কমন ৷ তা হলো, রোগ নির্নয় এবং চিকিৎসা ৷ আর যারা রোগ নিরিনয় এবং চিকিৎসা করেন তারা সিকিৎসক বা ডাক্তার ৷ সুতরাং ফিজিওথেরাপিষ্টরাও ডাক্তার ৷ আর ডাক্তাররা নামের পূর্বে ডাঃ প্রিফিক্স লিখবে এটা পানির মতন সরল কথা ৷
এখন আসি বিএমডিসি প্রসঙ্গে ৷ বিএমডিসি MBBS এবং BDS ডিগ্রীধারীদের কাউন্সিল ৷ তাদের কোন অধিকার নাই ফিজিওথেরাপি বা অন্যকোন হেলথ প্রফেশনকে নিয়ন্ত্রণ করার ৷ MBBS বা BDS ডিগ্রীধারী ছাড়া অন্য কেউ নামের আগে ডাক্তার লিখবে না এমন আইন পাস করা অনধিকার চর্চা ছাড়া আর কিছুই না ৷ এখন প্রশ্ন হলো সদ্য সরকারি নিয়োগ পাওয়া হোমিওপ্যাথি, হার্বাল ও ইউনানী সিকিৎসকরা ডাক্তার হলো কিভাবে? উপজেলা পর্যন্ত সকল হাসপাতালে তাদের সরকারি নেমপ্লেটে তো নামের পূর্বে ডাঃ ই লেখা ৷ আসলে তাদের নামের পূর্বে ডাক্তার লেখার জন্য মন্ত্রনালয় থেকে বিশেষ প্রজ্ঞাপন জারি করা আছে ৷ একইভাবে ফিজিওদের একটা প্রজ্ঞাপন আছে এমন যে, “ফিজিওথেরাপি চিকিৎসকদের জন্য সতন্ত্র কাউন্সিল তৈরি প্রক্রিয়াধীন এবং কাউন্সিল তৈরি না হওয়া পর্যন্ত ফিজিওথেরাপি চিকিৎসকরা নামের পূর্বে ডাক্তার লেখে সারা দেশে প্রাকটিস করতে পারবে এতে তাদের কোন হয়রানি করা যাবে না ৷” এবং বিএমডিসি’র ডাক্তার লেখার এই ধারাটা মহামান্য হাইকোর্ট কতৃক ফিজিওথেরাপি চিকিৎসকের উপর স্থগিত আছে ৷ সুতরাং নামের পূর্বে ডাক্তার লিখতে বিএমডিসি এখন আর ফিজিওদের জন্য কোন বাধা নয় ৷
অনেকে বলেন বিদেশে তো ফিজিওথেরাপিস্ট রা নামের পূর্বে ডাক্তার লেখেন না ৷ কথা ঠিক, তবে জেনে রাখা জরুরি যে সেসব অধিকাংশ দেশেই অন্যান্য সিকিৎসকরাও নামের পূর্বে ডাক্তার লেখেন না ৷ তারা সাধারনত ফিজিশিয়ান, নিউরোলজিস্ট, অর্থোপেডিশিয়ান, ইউরোলজিস্ট, গাইনোকোলজিস্ট ইত্যাদি ইত্যাদি নামে পরিচিত ৷ আমাদের পার্শবর্তী দেশ ভারত, পাকিস্তান, সৌদি আরব, ইরাক, ইরান, কুয়েত, কাতার ইত্যাদি অনেক দেশে ফিজিওরা নামের পূর্বে ডাক্তার লেখেন ৷ আর বাংলাদেশেও যেহেতু চিকিৎসকদের নামের পূর্বে ডাক্তার লেখা একটা প্রচলন তাহলে ফিজিওথেরাপি চিকিৎসকদের নামের পূর্বে ডাক্তার লিখতে সমস্যা কোথায়?
এখন প্রশ্ন আসতে পারে ফিজিওরা ডাক্তার হওয়ার মতো এমন কি পড়াশোনা করে? ঢাকা বিশ্ববিদ্যালয়ের মেডিসিন ফ্যাকাল্টির অধীনে ফিজিওথেরাপি ৫ বছর মেয়াদী (৪ বছর একাডেমিক + ১ বছর ইন্টার্নী) একটি প্রফেশনাল কোর্স ৷ চারটা পেশাগত পরীক্ষায় তাদের মোট ৬২৫০ মার্কসের পরীক্ষায় তাদের উন্নীত হতে হয় ৷ চারটা প্রফে তারা ৩৩টা সাবজেক্ট পড়ে ৷ এর মধ্যে আছে এনাটমি (৪০০ মার্কস, ২ বছর), ফিজিওলজি (৪০০ মার্কস, ২ বছর), বায়োকেমিস্ট্রি(২০০ মার্কস, ১ বছর), প্যাথলোজি এবং মাইক্রোবায়োলজি (৩০০ মার্কস, ২ বছর), জেনারেল সার্জারি (১৫০ মার্কস, ১ বছর), ফার্মাকোলজি (৩০০ মার্কস, ২ বছর), অর্থোপেডিক (১৫০ মার্কস, ১ বছর), পেডিয়াট্রিক(১৫০ মার্কস, ১ বছর), নিউরোলজি(১৫০ মার্কস, ১ বছর), কার্ডিওপালমোনারি (১৫০ মার্কস, ১ বছর), কমিউনিটি মেডিসিন(১০০ মার্কস, ১ বছর) , সাইকোলজি (১০০ মার্কস, ১ বছর), সাইকিয়াট্রি(১০০ মার্কস, ১ বছর), রিসার্চ প্রজেক্ট বা থেসিস (২০০ মার্কস, ১ বছর, ৩০০ ঘন্টা) সহ ফিজিওথেরাপি চিকিৎসা সম্পৃক্ত আরো ১৯টা সাবজেক্ট ৷ সাথে নিয়মিত ল্যাব এবং ক্লিনিকাল প্রাকটিস তো আছেই ৷ এমন সিলেবাস ডাক্তার হওয়ার জন্য সারা পৃথিবীতে গ্রহনযোগ্য এবং যথেষ্ট মান সম্পন্ন ৷
অনেকের প্রশ্ন, ফিজিওথেরাপি চিকিৎসকরা কি ওষুধ লিখতে পারেন? আসলে ফিজিওথেরাপি মূল চিকিৎসা হচ্ছে ফিজিক্যাল মেথোডে ৷ অর্থাৎ ওষুধ বিহীন চিকিৎসা ৷ তবে যুক্তরাজ্য সহ কিছু দেশে ফিজিওথেরাপিষ্টরা ওষুধ প্রেসক্রাইব করার জন্য স্বীকৃত ৷ বাংলাদেশর ফিজিওথেরাপিষ্টরাও ওষুধ প্রেসক্রাইব করার জন্য ফার্মাকোলজিতে যথেষ্ট দক্ষ ৷ তবে ফিজিওদের কোন কাউন্সিল না থাকায় বাংলাদেশে ফিজিওরা ওষুধ প্রেসক্রাইব করবে কিনা বা করলেও তাদের সীমাবদ্ধতা কতোটুকু তা বিচারের মানদন্ড এখোনো নির্ধারিত হয়নি ৷ তবে ফিজিওরা ইনফিলট্রেশন বা ইনজেকশন দিতে পারেন, এটা সারা পৃথিবীতে স্বীকৃত ৷
সাস্থ্য বিভাগের কোন অংশকে দমিয়ে রেখে কখনো ভালো সাস্থ্য সেবা দেওয়া সম্ভব নয় ৷ প্রয়োজন সামগ্রিক উন্নয়নের ৷ তাহলেই রোগীরা উপকৃত হবে ৷ দেশের সিকিৎসার উপর মানুষের আস্থা বাড়বে ৷ চিকিৎসার জন্য দেশের বাইরে যাওয়ার প্রবনতা কমবে ৷ সঠিক তথ্য এবং চিকিৎসা মানুষের মৌলিক অধিকার ৷ নিজে সচেতন হউন, সঠিক তথ্য জানুন, অন্যকে জানান ৷

Graduate physiotherapist ( BPT/BScPT Degree holder from faculty of medicine in a recognised University) should be used ( Dr ) prefix because they are qualified from faculty of medicine University of Dhaka, same as MBBS & BDS. where as A Engineering faculty have so many subject like mechanical engineering, Electrical engineering, civil engineering, Computer engineering, Auto mobile engineering etc. If anybody who complete BSc Engineering in any discipline from faculty of engineering he/she become Engineer and get all facilities . But BPT ( Bachelor of physiotherapy), MBBS (Bachelor of medicine & surgery), BDS (Bachelor of Dental surgery) are the 3 subject of faculty of medicine University of Dhaka 2 subject (MBBS & BDS) get all the facility like BMDC registration, BCS (Health), Govt – post in every hospital etc. But another one BPT is neglected in everywhere, They’ve no council, They can’t appear at BCS (Health), They’ve no post in any Government hospital. why ??????

বিএমডিসি এক্ট ২০১০ প্রসঙ্গেঃ
বিএমডিসি এক্ট ২০১০ একটি সুন্দর আইন- শুধুমাত্র এমবিবিএস আর বিডিএস ডিগ্রীধারীদের জন্যে। যারা জিপি (জেনারেল প্র্যাক্টিশনার হিসেবে) প্র্যাক্টিস করেন- শুধুমাত্র তাদের জন্যেই এই আইন প্রযোজ্য হওয়া উচিৎ।
ফিজিওথেরাপিস্ট (নুন্যতম ব্যাচেলর ডিগ্রীধারী) দের জন্যে সরকার যেহেতু কোনো কাউন্সিল বা বোর্ড গঠণ করেনি, আর তারা বিএমডিসি এর সদস্য ও নয়, তাই তারা বিএমডিসি এক্ট ২০১০ মানতে বাধ্য নয়।
তবে, যে কোনো পেশার, যে কেও ম্যালপ্র্যাক্টিস করলে- তাকে আইনের আওতায় আনা, সেটা সরকারেরই দায়িত্ব হওয়া উচিত। কোনো ফিজিওথেরাপিস্ট ও যদি জিপি প্র্যাক্টিস করেন, তবেই কেবল বিএমডিসি এক্ট ২০১০-এর ক্রিমিনাল সেকশন -তার উপর প্রযোজ্য হতে পারে। তেমনি ভাবে, অন্য পেশার কেও যদি ফিজিওথেরাপি বিশেষজ্ঞ সাজেন, তবে সেটাও হবে আইন বহির্ভূত, যেটা এই সোনার বাংলায় অহরহ দেখা যায়।
নামের আগে ডাঃ লেখা বা না লেখা-এটা বাংলাদেশের প্রেক্ষিতে একটি সামাজিক ব্যাপার। দেশে শিক্ষা-সচেতনতার হার বাড়লে এই ফালতু ব্যাপার নিয়ে ফিজিওথেরাপিস্ট রা মাথা ঘামাবেনা, ইংশা-আল্লাহ।

of course Diploma holder i mean DMF should write Dr. prefix their name. they cover a large portion of medical sector. a lion portion of people depend to them and they perform very well as well as the people are also satisfied with them, after three years academic course with one year internship they go through most of the part of general medical science. they are skilled enough to treat , manage , and assess in the primary Health care system. so as many people think they have the right to use Dr. before their name. it”s their basic right to write Dr. as well as higher Education.
